Speech-Lanagauge Services
At Jacksonville Speech & Hearing Center, we provide services to help individuals of all ages develop better speech and language skills and to educate their loved ones on how to promote speech and language skills in the home. We provide diagnostic evaluations and treatment of speech and language problems including:
- Oral motor speech disorder
- Receptive and expressive language disorder
- Voice disorders
- Stuttering/disfluency
- Apraxia
- Literacy/dyslexia
- Articulation/phonological disorders
Audiology Services
Early detection and treatment shows a reduction in the occurrences of many of these other health-related concerns. It’s never too early or too late to take steps to address a hearing concern. At Jacksonville Speech & Hearing Center, we are proud to offer a variety of audiology services including:
- Pediatric and adult diagnostics
- Hearing aid prescribing, fitting, and adjusting
- Otoacoustic Emission (OAE)
- Tinnitus Treatment
- OSHA compliant industrial screenings
- Ear molds for hunting, recreational shooting, swimming, musicians, and hearing aids

Hearing forms are listed together and speech-langauage forms are listed together, followed by other miscellaneous forms.
Audiology/Hearing - Adult
This for is for individuals over the age of 18, coming to the clinic for a routine or comprehensive hearing evaluation.
This for is not for veterans being sent to us through a third party scheduler, with the exception of the VA Community Care Network. For veteran exit exams, please see a different link below.
Audiology/Hearing - Pediatric
This form is for patient under the age of 18 coming in for a hearing screening for comprehensive diagnostics. PLEASE NOTE: If your child is under 3 years of age, it is important that you schedule the appointment during a nap time. We do not do sedated testing and they will need to be able to sit or lay quietly.
Tinnitus/Ringing
These forms are a separate set of forms that should be completed if you are experiencing grinning in your ears and would like to discuss treatment options during your visit.
If you are completing these form, you still need to complete the pediatric or adult hearing form.
Speech-Language - Pediatric
This is for individuals under the age of 18, coming to the clinic for a comprehensive speech-langauge evaluation.
This form is not for adults or individuals who need a swallow test.
Speech-Language - Adult
This is for individuals 18 years of age and older, coming to the clinic for a comprehensive speech-langauge evaluation.
This form is not for individuals who need a swallow test.
Telehealth Forms
If you plan to receive services through one of our telehealth options, this form is needed.
This form is to be completed in addition to the other form(s) you need.
